RN Case Manager - Home Health

Grace Home Healthcare, a HomeCentris Healthcare Company, is seeking an experienced home health RN Case Manager for our Personal Care division in Fairfax, Virginia to provide quality care and nursing oversight of its non-skilled home care services in the setting where our clients most want to be, in their homes.

The RN Case Manager will assist in providing the overall management and supervision of client care activities within the home care setting. The RN utilizes a general understanding of the principles of nursing and basic physical assessment skills to plan, coordinate, provide and document care for an assigned caseload of patients from day of admission through day of discharge. Utilizing physicians’ orders and professional skills, the RN participates in development and implements a home health plan that meets each patient’s specific needs and is in compliance with federal and state regulations, reimbursement guidelines as well as agency policies and procedures.


Coordinating care to assigned clients, establishing a goal-directed care plan from admission (Start of Care) to discharge, including comprehensive ongoing assessments of clients’ needs.
Perform routine onsite supervisory visits to assess client, family, environment, and clinical caregivers and complete timely follow-up documentation
Ensure availability and proper operation of necessary equipment and supplies related to patient care.
Perform quarterly nurse documentation audits
Evaluate the quality and effectiveness of nurse practice and nursing services, analyzing appropriate data and information to identify opportunities for collaboration with all stakeholders in order to improve services and patient outcomes.
Provide guidance, supervision, orientation, training, education, and evaluation of clinical field staff.Provide excellent customer service.
Provide nursing updates and obtain re-authorization for continued care. Write ISPs and quarterlies for DD-waiver clients utilizing WAMs system.
Maintain compliance in accordance with company policies and procedures, laws and regulations, and professional standards within the state of practice. Knowledge of DMAS is preferred.
Maintain a professional demeanor consistent with registered nurse standards of practice.
Perform other duties as assigned by supervisor.

RN license ability to practice in Virginia
Covid-19 vaccine (Proof)
CPR or BLS certification

Home Health experience
DD waiver experience
CCC plus/Medicaid experience
DMAS regulations experience
